Up to [gforth] / gforth

made classical 1-register stack caching work again (and probably
  general stack caching with non-empty canonical state).  There was a
  subtle bug involving ?DUP-?BRANCH.
made this the default for gforth-fast and turned off anything beyond
deleted the now-unused rewrite_bb().
