Up to [gforth] / gforth

Added automatic glossary entry transfer from primitives to the texi file.
renamed gfoprth.texi to gforth.ds.
fixed a few minor bugs.
changed the behaviour of locals scoping when encountering an unreachable BEGIN.
made UNREACHABLE immediate
